    The most common cause for out of focus images is a mismatch in the mount between camera and lens.

    However in this case, you have a C-Mount camera and a C-Mount lens.

    One possibility is that your camera or lens accidently have a C-CS mount adapter installed, such as the one found at CS to C Mount 5mm Spacer Adapter | Teledyne FLIR.  Sometimes they are stuck and seem part of the camera or lens.

    Another option is that the working distance is too close.  Please move the camera further away from the subject to see if it can focus better.  If the camera can then focus, choosing a different lens for a shorter working distance, or even using a 1mm shim, may help focus better at the shorter distance.

    Increasing the depth of field by reducing the aperture (and possibly increasing your lighting) may also help focus if the issue is a depth of field issue.
